Breast cancer in men

Breast cancer in men is the same disease as that which afects women. Both men and women have breast tissue, although men have less breast tissue than women. Most of the breast tissue in men is located behind the nipple.

Male breast cancer can be early or advanced at diagnosis. Early breast cancer is cancer that is contained in the breast and may or may not have spread to the lymph nodes in the breast or armpit.

The most common breast cancer found in men is invasive ductal carcinoma. This cancer typically presents as a lump. However there are other symptoms that men should be aware of.

Can men really get breast cancer?

It may come as a surprise to most men to learn that they can develop breast cancer. While breast cancer is uncommon in men, it’s important for men who fnd a change in their breasts not to let embarrassment or uncertainty prevent them from seeing their doctor without delay. Early detection and treatment are the best way to survive the disease.

Types of breast cancer in men

Ductal carcinoma in situ (DCIS)

In DCIS (also known as intraductal carcinoma), cancer cells form in the breast ducts but do not grow through the walls of the ducts into the fatty tissue of the breast or spread outside the breast. DCIS accounts for about 1 in 10 cases of breast cancer in men. It is almost always curable with surgery.

Infiltrating (or invasive) ductal carcinoma (IDC)

This type of breast cancer breaks through the wall of the duct and grows through the fatty tissue of the breast. At this point, it can spread (metastasize) to other parts of the body. At least 8 out of 10 male breast cancers are IDCs (alone or mixed with other types of invasive or in situ breast cancer). Because the male breast is much smaller than the female breast, all male breast cancers start relatively close to the nipple, so they are more likely to spread to the nipple. This is different from Paget disease as described below.


Infiltrating (or invasive) lobular carcinoma (ILC)

This type of breast cancer starts in the breast lobules (collections of cells that, in women, produce breast milk) and grows into the fatty tissue of the breast. ILC is very rare in men, accounting for only about 2% of male breast cancers. This is because men do not usually have much lobular tissue.

Lobular carcinoma in situ (LCIS)

In LCIS, abnormal cells form in the lobules, but they do not grow into the fatty tissue of the breast or spread outside the breast. Although LCIS is sometimes grouped with DCIS as a type of non-invasive breast cancer, most breast specialists think it is a risk factor for developing breast cancer rather than a true non-invasive cancer. As with invasive lobular carcinoma, LCIS is very rare in men.

Paget disease of the nipple

This type of breast cancer starts in the breast ducts and spreads to the nipple. It may also spread to the areola (the dark circle around the nipple). The skin of the nipple usually appears crusted, scaly, and red, with areas of itching, oozing, burning, or bleeding. The fingertips can be used to detect a possible lump within the breast.

Paget disease may be associated with DCIS or with infiltrating ductal carcinoma. It accounts for about 1% of female breast cancers and a higher percentage of male breast cancers.

Inflammatory breast cancer

Inflammatory breast cancer is an aggressive, but rare type of breast cancer. It causes the breast to be swollen, red, warm and tender rather than forming a lump. It can be mistaken for an infection of the breast. This is very rare in men.

It is not possible to say exactly what causes breast cancer in men. However, research has shown that there are some things that increase a man’s chance of developing breast cancer. These are called ‘risk factors’.

The most common risk factors are:
  • getting older - breast cancer in men occurs more commonly in those aged 50 years and older
  • having a strong family history-having a family history of female or male breast cancer or ovarian cancer on either side of the family can increase your risk of developing breast cancer.
However, most men who develop breast cancer do not have a strong family history of the disease. If you are concerned about your risk based on family history see your GP.

Other less common factors that may increase risk include:
  • high oestrogen levels
  • Klinefelter’s syndrome
  • some testicular disorders
  • radiation exposure.
The triple test includes:
  • clinical breast examination and taking a personal history
  • imaging tests i.e. mammography and/or ultrasound
  • non-surgical biopsy i.e. a fne needle aspiration and/or core biopsy.
A clinical breast examination involves checking both breasts and feeling the lymph nodes under the armpits. Imaging tests involve an X-ray of the breast, called a mammogram, or a scan that uses sound waves to produce an image of the breast, called an ultrasound.

If the imaging test shows an abnormal area, you may have a biopsy. This involves taking a sample of cells or tissue from the breast and examining them under a microscope.

What Are the Causes of Prostate Cancer?

Prostate Cancer (PC) is one of the most common diseases which has been "hitting" men aged 50 and above. It's exact causes are unknown, yet there have been noted contributing factors which have been pointed out by some research as they have allegedly been affecting how cancer cells tend to spread and grow. These are:

Age. Men over 50 are at risk of Prostate Cancer and the risk correspondingly increases with age. This doesn't mean though that men aged below 50 are safe from this disease. They too can get it but the probability is lower compared to those aged 50 above.

Testosterone. The testosterone (male hormone) level can affect the risks of having Prostate Cancer. Though testosterone itself does not cause PC, it would feed its growth, which is the very reason why some cancer treatments are aimed to block the body's production of testosterone.

Hereditary Factors. Like in any other types of cancer diseases, hereditary factors play a significant role in developing Prostate Cancer. Men who have Prostate Cancer-stricken relatives are at a higher risk of developing it than men whose family doesn't have any trace of relative having PC.

Diet. This factor is said to contribute so much to the development of Prostate Cancer. Yet this is the most controllable one since it is influenced by one's food intake which must be free from animal fats, which leads to a greater risk of PC.

Given the contributing factors leading to Prostate Cancer, one may ask if there could be a way to avoid these risks or more so, keep himself from developing the disease. There may not be anything we can do about age and hereditary factors, so focus must be made on diet and testosterone, which are the controllable factors.

There are only three basic steps to prevent Prostate Cancer. First, watch your diet. Avoid dairy food or those which have a high content of animal fats. Work on a prostate-friendly diet. Second, change your
lifestyle. Engage in a healthy lifestyle. Do some regular exercise which could keep you and your prostate fit and healthy. Finally, take some nutritional supplements. Your daily intake of food may not serve you the adequate and appropriate nutrients you might need to maintain a healthy prostate. Go natural. Cancer is a condition that none of us would ever want to have. No matter how healthy you think you are, there is always that remote possibility that you would contract cancer. In the case of prostate cancer, men are obviously the ones that are affected. In this article, we would be discussing the basics of prostate cancer, it's most common signs and symptoms and the diagnostic procedures employed to identify the condition.

Cancer of the prostate gland happens when cancer cells develop in the prostate. This is a gland found at the bottom of the urinary bladder. A normal sized prostate would be around the size of a walnut. As men grow older, it is normal for the prostate to also enlarge. However, abnormal enlargement of the prostate that is caused by imbalances in the hormones is called Benign Prostatic Hyperplasia or BPH. This is different from prostate cancer in a way that BPH can be operated on and does not metastasize. Treatment for prostate cancer would involve the use of surgery, chemotherapeutic drugs and/or radiation.

In prostate cancer, cancer cells gather in the area and slowly grow into a cancerous tumor. The cancer cells might have been there for a while before any symptoms become evident. However, not all symptoms are similar and not all cancer cells act the same way. There are those that proliferate fast, while there are others that progress more slowly. The aggressiveness of cancer growth can be measured by the Gleason Score.

Though both cancer of the prostate gland and benign prostatic hyperplasia increase the size of the prostate, medical findings state that they are not related. Factors that might predispose a person to getting prostate cancer would be the following: age, gender, genetics, race, hormones and exposure to toxins.

It is best to know which prostate cancer symptoms to watch out for so that you would be able to get consultation at the soonest time possible. A person with suspected prostate cancer would be found to have a hard nodule in the prostate area during a routine digital rectal exam. In some cases, where the cancer is in its advanced stages, the prostate may become too enlarged that they would impede the flow of urine. For others, they would notice blood in their urine, also called hematuria. This bleeding is painless, a very common sign of cancer. These are the most common symptoms of cancer and this usually prompts patients to seek consultation.

Screening for cancer of the prostate gland would involve getting blood work done. The technologist would be looking for the cancer marker PSA or prostate specific antigen. A result that is higher than 4ng/ml is usually observed in people with prostate cancer.

A digital rectal exam was mentioned earlier. During this exam, an enlarged prostate would be felt by the examiner, and this would prompt for further evaluation.

A biopsy is needed to make an accurate conclusion of prostate cancer. A tissue sample would be sent to the laboratory and they would be testing the sample for cancer cells.

Lung cancer is caused by uncontrolled rapid growth of cells in tissues. This type of cancer is most common and results in more than a million deaths every year. This form of cancer is indicated by weight loss or coughing up blood or regularly going out of breath. It can be noticed on chest radio graph also called CT Scan. The treatment that one gets depends on the stage that one is in. Treatment of cancer include surgery, chemotherapy and radiotherapy.

CLASSIFICATION

Lung cancers are classified after studying under them microscope. Classification is necessary as different type of cancer is treated differently. Large portion of lung cancer are carcinomas - malignancies that grow from epithelial cells. Lung-carcinomas are categorized into two types: non -small and small-cell lung carcinoma. Non-small cell lung carcinoma and small cell lung carcinoma account for 80. 4% and 16. 8% frequency of lung cancer, respectively.

1. NON -SMALL CELL LUNG CARCINOMA

The non -small cell lung carcinomas are grouped together as their prognosis and management are same up to some extent. They are further classified into three types: squamous cell lung carcinoma, adenocarcinoma and large cell lung carcinoma. Squamous cell lung cancer originates near a central bronchus. They account for 25% of lung cancers. Adenocarcinoma starts in peripheral lung tissue. The cases of adenocarcinoma are a result of smoking. They accounts to 40% of non -small cell lung cancers.

2. SMALL CELL LUNG CARCINOMA

This type of lung cancer is rare. It is sometimes referred to as "oat cell" carcinoma. Most of the times they originate from larger airways (primary and secondary bronchi ) and from there they grow at a rapid pace. This type of lung cancer if mostly associated with smoking.

SECONDARY CANCERS

These cancers are classified on the basis of site of origin like breast cancer but has spread to the lung. Majority of the lung cancers in children are secondary.

STAGING OF LUNG CANCER

Lung cancer staging is used to asses the degree of spread of the cancer from its place of origin. It is an important factor that determines the potential treatment of lung cancer. The degree starts from 1A to 4, 1A being best prognosis and 4 being worst.

SIGNS AND SYMPTOMS

Following are the symptoms of lung cancer: 1. Voice becoming hoarse. 2. Sudden loss of weight. 3. Feeling pain in chest region or abdomen. 4. Difficulty in swallowing. 5. Loss of appetite. 6. Running out of breath. Many of the symptoms mentioned above are non -specific. By the time they notice symptoms or signs, cancer has already spread from place of origin. Very few people with this cancer have signs at time of diagnosis, these cancers are noticed on routine chest radio graph.

CAUSES

The three main causes of cancer are: carcinogens (which is found in tobacco ), viral infection and ionizing radiation. If exposed, it causes changes to DNA in tissue lining the bronchi of the lungs. With more and more tissues getting damaged, cancer develops.

1. SMOKING

Smoking is the main cause of cancer. In one cigarette, there are 60 different known types of carcinogens like radioisotopes and nitrosamine. Smoking is believed to cause 80% of these type of cases. The risk is generally less in non -smokers. The time that a person smokes proportionately increases the chances of this cancer. There has been cases that if a person stops smoking, the damaged cells gradually gets repaired. In non-smokers, passive smoking is the main causes of lung-cancer. Passive smoking is one inhaled from another person smoking.

2. RADON GAS

The gas produced from breakdown of radium. This gas is colourless and odorless. Exposure to radiation ionize the genetic material, causing mutations that sometimes turn cancerous. Exposure to radon gas is the second major cause of lung-cancer after smoking.

3. ASBESTOS

Asbestos is responsible for causing a number of cancer, one among them is lung cancer. In UK, asbestos accounts for 2 to 3% of the total cases of this cancer.

4. VIRUS

Viruses are responsible for causing lung-cancer in animals. And research has shown of similar potential in humans.

5. PARTICULATE MATTER

Particulate matter has a direct link to lung cancer cases. The size and quantity of particles in air determines the risk of getting lung-cancer. If concentration of particles increases beyond 1%, then the chances of getting this increases by 14%.

PATHOGENESIS

Just like may other cancer forms, lung cancer is started by activation of ocnogenes or inactivation of tumor suppressing genes. Ocnogenes are those genes that make people more vulnerable to cancer. Ocnogenes are produced from proto-ocnogenes, when the latter is exposed to particular carcinogens. In k-ras proto-oncogene, mutations takes place which are responsible for 10 to 30% of lung adenocarcinomas. Tumor invasion, angiogenesis, apoptosis, cell profileration are regulated by the Epidermal growth factor receptor. Mutations and amplification of EGFR are common in non -small cell lung cancer. The basis for treatment with EGFR-inhibitors are also provided by Mutation and amplification of EGFR. Chromosomal damage can lead to loss of heterozygosity which can result in inactivation of tumor suppressor genes. Damage to four of these chromosomes:3p, 5q, 13 q and 17 p are common in small cell lung-carcinoma. The p53, which is a tumor suppressor gene, located on chromosome 17p is affected in most of the cases. c-MET, NKX2-1, LKB1, PIK3A and BRAF are also mutated or amplified. Various genetic polymorphisms are supplementary to this cancer. Some of them include polymorphisms in genes coding for interleukin-1, cytochrome p450, apoptosis promoters such as caspase-8, and XRCC1, which is DNA repair molecule. People having these polymorphisms are more likely to develop lung cancer on being exposed to carcinogens. The research has revealed that MDM2 309G allele is a low-penetrant risk factor for developing this in Asians.

DIAGNOSIS

If a person has reported symptoms that might suggest cancer related to lungs, then chest radio graph is performed in the first step. The test reveals the widening of mediastinium, atelectasis and pleural effusion. Even if there are no radio graphic findings but the hint of this is high because of things like the person being heavy smoker with blood-stained sputum then CT-Scan may provide the necessary data. If findings are unnatural in cells in sputum, then they multiplies the risk of this type of cancer. Early detection can be done by Sputum cytologic examination together with other screening examinations. The differential diagnosis for those patients who show irregularities on chest cardiograph consider cancer related to lungs along with non malignant diseases. These consider infectious reasons like tuberculosis or pneumonia. The above mentioned diseases can lead to lung nodules.

PREVENTION

Prevention, just like always, is better than cure. Steps in this direction have been taken by may countries by identifying carcinogens and banning them but tobacco, which is the major cause of lung cancer, is still common. Eliminating cigarette smoking is first hand target in the prevention of lung cancer. Steps to lessen Passive smoking have also being taken by banning smoking in public places and workplaces. New Zealand has restricted smoking in open places. A similar step is also taken by Chandigarh, India. Bhutan has criminalized smoking since 2005.

SCREENING

Screening is used to detect disease by doing medical tests when the patient is not showing any symptoms. Chest radio graph or computed tomography are the tests used for screening of lung cancer. But, results have shown, that screening tests for lung cancer rarely has shown any benefit.

TREATMENT

The treatment of lung cancer can be done in following ways, depending on the stage or degree of cancer:

1. SURGERY

If doctors have detected lung cancer, then CT scan and positron emission tomography are usually applied to check if the disease is placed and surgery can be done or it has moved to the point where performing surgery is not possible. Surgery can only be performed if spirometry reveals good respiratory reserve, but if it is poor, then surgery is not possible. Even surgery has a death operative rate of 4. 4% but that is because of patient's lung function and other factors.

2. CHEMOTHERAPY

Chemotherapy, along with radiation, is used to treat small cell lung carcinoma. Primary chemotherapy is also used in metastatic non -small cell lung carcinoma.

3. RADIOTHERAPY

Radiotherapy, with chemotherapy, is given when patient is not fit to under go surgery. This type of high intensity radiotherapy is called radical radiotherapy. CHART (continuos hyperfractioned accelerated radiotherapy ) is refined version of this technique in which a high dose of radiotherapy is given for a short period of time. When cancer affects a short section of bronchus, then brachytherapy is given.

EPIDEMOLOGY

Lung cancer is the most widely reported cancer. There are 1. 35 million cases every year and 1. 18 million deaths. Lung cancer develop among those who have a history of smoking over a long period of years i. e 50 years and above. In addition to smoking, passive smoking is also a factor that causes lung cancer. Even the emissions from factories, automobiles, power plants pose a threat to human health. Lung cancer is found to have a reciprocal effect with sunlight and UVB exposure. How Asbestos Causes Mesothelioma - New Hope For Prevention and Treatment

Two researchers at the University of Hawaii Cancer Research Center have found the causal pathway that leads from the presence of asbestos fibers in the lung to the development of the aggressive and deadly cancer mesothelioma. In a study published in the Proceedings of the National Academy of Sciences, Drs Haining Yang and Michele Carbone demonstrated that the microscopic asbestos fibers which become embedded in lung tissue trigger an inflammatory process that leads to cell mutations. They identified a molecule, HMGB1, which cells release when they are exposed to asbestos. HMGB1 regulates the inflammatory response, triggering the chronic inflammation that promotes tumor growth and over time develops into mesothelioma.

The importance of their work is the confirmation of this causal pathway and understanding of HMGB1's involvement. In order to design treatment and prevention strategies, doctors must first understand the causal mechanism, so they know what processes to target and how.

Inflammation has long been understood as a contributor to disease processes as varied as heart disease, Alzheimer's, diabetes, and arthritis. Its involvement in other cancers has been well documented. Recently, researchers have shown that regular consumption of aspirin, which is an anti-inflammatory agent, can slow the growth of colon cancer.

The challenge for prevention of mesothelioma is that once the microscopic asbestos fibers are inhaled and make their way into lung tissue, it is impossible to remove them. The focus instead must be on reducing the embedded fibers' potential to cause inflammation and therefore disease. Drs Yang and Carbone now believe that by focusing on HMGB1 they have a means to reduce the chronic inflammation. This could lead both to preventing the development of mesothelioma, and for people who already have the disease, slowing its growth. Because mesothelioma is a slow-growing cancer, a treatment that could retard that growth process could prolong lives and significantly improve quality of life.

The work that led to these findings was conducted on mice and hamsters, whose lung tissue responds to asbestos very similarly to humans' lungs. Their next study will be with humans, in a region of Turkey where the death toll from mesothelioma is the highest on earth. In rural Anatolia and southeastern Turkey, where the soil is saturated with asbestos, and villagers plaster their houses with an asbestos compound, nearly half the population dies of mesothelioma. Drs Yang and Carbone plan to test a variety of medications, including aspirin, to gain a better understanding of the inflammatory process, and how to control it. Their findings are likely to provide new pathways for treatment of this aggressive cancer.
